As we are part of a Primary Care Network, we have 2 pharmacists supporting our practice.  Yasmin and Andy are in house on alternate Thursdays and Fridays.

Their role within general practice is patient facing to provide clinical medicine leadership both in running pharmacist practice clinics and supporting general practice staff. They aim to:

  • Undertake structured medication reviews for patients facing long term conditions to support patients with their medications to ensure they get the best use of their medicines
  • Improve medicines optimisation and safety to reduce the risk and harm from poor use of medicines with the aim to reduce unplanned hospital admissions
  • Improve antimicrobial stewardship prescribing in line with local and national guidance and recommendations
  • Supporting care home residents formulating care plans to improve medicine management
  • Manage minor ailments by undertaking differential diagnoses within my scope of practice and develop my diagnostic and clinical assessment skills to become a fully embedded clinician with the surgery
  • Provide specialist advice to other pharmacist colleagues in other sectors 

Community Midwives

The midwife provides shared care and advice during the antenatal and postnatal periods. She may be contacted at the Maternity Office, Horsham Hospital 01403 227000 ext 7134. An antenatal clinic is held on Tuesday afternoons at Cowfold surgery. Contact Reception for an appointment.

Community Nurses

The District Nurses provide nursing treatment and advice to our housebound patients and those recently discharged from hospital. They mey be contacted on 01293 228311. This is provided by One Call.

Health Visitors

Cowfold Medical Group are part of Horsham South Healthy Child Programme (HCP) who provide services from pregnancy to age 19.

The HCP team can support children, young people and families with a variety of issues including:

  • Feeding and nutrition
  • Behaviour and sleep management
  • Parenting issues
  • Maternal mental health
  • Emotional health
  • Child protection issues

The HCP team consist of Specialist Community Public Health Nurses (nurses/midwives with an additional degree level qualification in public health and often referred to as health visitors and school nurses), registered nurses, nursery nurses, school health assistants and administrative support.
